چکیده

Refractory hypoxemia is an extremely complex condition with a high morbidity-mortality rate. This clinical situation represents an advanced process encompassed within the so-called acute respiratory distress syndrome (ARDS), defined by the American-European Conference of 1994 with the purpose of reaching consensus on a series of homogeneous criteria. Acute respiratory failure (ARF) is defined on the basis of clinical, radiological and blood gas parameters. Acute lung injury (ALI) in turn is defined as a PaO2/FiO2 ratio of under 300 mmHg, and in ARDS it is taken to represent a ratio of under 200 mmHg. Recently, in the year 2011, experts gathered in Berlin and redefined the classification (the ‘‘Berlin definition’’), improving stratification and mortality prediction of the syndrome, but without clarifying

Acute Respiratory Distress Syndrome in a Patient With Suspected Influenza: A Case Report

Acute Respiratory Distress Syndrome (ARDS) is a clinical syndrome consisting of tachypnea, refractory hypoxemia, and diffuse opacities on chest radiographs after infection or trauma which ultimately leads to respiratory failure. The principles of treatment are based on patient care in ICU, mechanical ventilation and medical treatments. By using lower plateau, less tidal volume, higher positive ...

Therapies for refractory hypoxemia in acute respiratory distress syndrome.

Acute respiratory distress syndrome (ARDS) is a common and severe form of acute lung injury, resulting from both direct (eg, pneumonia) and indirect (eg, sepsis) pulmonary insults. It is a common cause of admission to the intensive care unit due to hypoxemic respiratory failure requiring mechanical ventilation, and is associated with significant morbidity and mortality. In some patients, ARDS l...

Approaches to refractory hypoxemia in acute respiratory distress syndrome: current understanding, evidence, and debate.

Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) cause substantial morbidity and mortality despite our improved understanding of lung injury, advancements in the application of lung-protective ventilation, and strategies to prevent ventilator-induced lung injury. Severe refractory hypoxemia may develop in a subset of patients with severe ARDS. We review several approaches...

Nonventilatory strategies for patients with life-threatening 2009 H1N1 influenza and severe respiratory failure.

Severe respiratory failure (including acute lung injury and acute respiratory distress syndrome) caused by 2009 H1N1 influenza infection has been reported worldwide. Refractory hypoxemia is a common finding in these patients and can be challenging to manage. This review focuses on nonventilatory strategies in the advanced treatment of severe respiratory failure and refractory hypoxemia such as ...
